    Service report template

What the technician found, what they did, what remains — captured on site and signed by the client before they leave.

    Who it is for

This template suits maintenance, repair and commissioning jobs. Its value lies in when it is filled: completed on site, it removes the evening re-keying and gives the client an immediate document rather than a promise to send one. It is deliberately short — a technician facing forty fields before leaving will fill the first ten properly and guess at the rest.

    The form structure

Section by section, in the order they are filled in the field.

    01Job

    • Client and sitechoice
    • Equipment concernedchoice or code scan
    • Job typechoice — preventive, corrective, commissioning
    • Arrival and departure timestime

    02Diagnosis and work done

    • Reported symptomlong text
    • Before photophoto
    • Work carried outlong text
    • After photophoto
    • Parts fittedmultiple choice, from stock

Linked to inventory, this field decrements stock with no second action.

    03Follow-up and sign-off

    • Job completechoice — yes, return needed
    • Reason for returntext, shown if a return is needed
    • Client signaturesignature

Timestamped and attached to the job. This is the record you look for six months later, if it is disputed.

Fields can be reordered, renamed and extended: this template is a starting point, not a fixed frame.

    What happens on submission

A form that stops at collection leaves the work undone. Here is what submission triggers.

    • A needed return creates the follow-up task, with its reason.
    • The signed report goes to the client and files itself in their folder.
    • Parts fitted update the stock.
    • Time spent feeds activity tracking by technician and by client.
