Quality audit form template

Who it is for
This template suits recurring internal audits — quality, safety, 5S, hygiene. It is built to be filled on mobile during the visit, not copied up afterwards. Scoring is deliberately simple: three levels are enough to sort, and a finer scale gets debated more than it gets decided.
